to ask again; to ask someone to repeat
To ask someone to say something again because you didn't hear or understand it the first time.
よく聞こえなくて聞き返した。
I couldn't hear well, so I asked again.
電話の声が小さくて、何度も聞き返した。
The voice on the phone was quiet, so I asked them to repeat several times.
聞き返すのが恥ずかしくて、わかったふりをしてしまった。
I was embarrassed to ask again, so I pretended I understood.

to ask back; to counter-question
To respond to a question with another question, turning it back on the person who asked.
「どう思う？」と聞かれて、「あなたは？」と聞き返した。
When asked "What do you think?", I asked back "What about you?"
質問に答えず、逆に聞き返すのは失礼だ。
It's rude to not answer a question and instead ask one back.
記者が鋭い質問をすると、大臣は笑いながら聞き返した。
When the reporter asked a pointed question, the minister smiled and asked a question back.
