Literally 'to exhaust one's goodwill.' 愛想 means friendliness or amiability, and 尽かす means to exhaust or use up completely. When someone 愛想を尽かす, they have run out of all patience and goodwill.

The person one is fed up with is marked by に: ～に愛想を尽かす. The passive form 愛想を尽かされる ('to have someone give up on you') is very common.
