To make a wish, dream, or hope become reality. Can describe both fulfilling one's own dreams and granting someone else's wishes.
(ゆめ)(かな)えたい。
I want to make my dream come true.
彼女(かのじょ)長年(ながねん)(ねが)いを(かな)えた。
She fulfilled her long-held wish.
()どもの(ころ)からの(ゆめ)(かな)えるために、毎日(まいにち)努力(どりょく)(つづ)けてきた。

The intransitive counterpart is (かな)う (to come true, to be fulfilled). (かな)える is transitive — the subject actively makes the dream or wish come true. Very common in inspirational and motivational contexts.
