ETYMOLOGY:
From German "Karte" (card). This is one of many medical terms in Japanese borrowed from German, reflecting the historical influence of German medicine on Japanese medical education during the Meiji era.

USAGE:
Used exclusively in medical contexts. Patients do not normally see their own カルテ directly, though they can request access. The word 診療録(しんりょうろく) is the formal legal term, but カルテ is used in everyday medical practice.
