行われる is the passive form, common for events
調査を行った上で、最終的な決定を出すことになっている。

This hospital conducts free health checkups every year, and they are used by local residents.
TRANSITIVITY: 他動詞 (transitive). Pattern: Xを行う (carry out X). No intransitive pair.
ASPECT: ている indicates ongoing action. 行っている = 'is conducting/carrying out.'
