In all of recorded history. Used as a modifier before superlatives to emphasize something as the most extreme or notable in history.

史上(しじょう)最高(さいこう)の記録(きろく)だ。

It is the highest record in history.

史上(しじょう)初(はじ)めての快挙(かいきょ)を成(な)し遂(と)げた。

They accomplished a historic first.

今年(ことし)の夏(なつ)は観測(かんそく)史上(しじょう)最(もっと)も暑(あつ)い夏(なつ)だったと発表(はっぴょう)された。

It was announced that this summer was the hottest on record.

Almost always appears before superlatives like 最高(さいこう) (highest), 最悪(さいあく) (worst), 最大(さいだい) (biggest), or 初(はじ)めて (first time). Very common in news headlines and sports commentary.
