An extreme state of bliss or pleasure where one loses awareness of surroundings. Often used for intense emotional or spiritual experiences.
恍惚(こうこつ)とした表情(ひょうじょう)を浮(う)かべた。
A look of ecstasy appeared on their face.
恍惚(こうこつ)状態(じょうたい)に陥(おちい)った。
They fell into a state of rapture.

2. senility; dementia (old usage)
An older usage referring to mental confusion associated with old age. Now less common but still found in literature.
祖父(そふ)は恍惚(こうこつ)の人(ひと)になってしまった。
My grandfather became senile.
『恍惚(こうこつ)の人(ひと)』 is also a famous novel title by 有吉佐和子(ありよしさわこ).
老齢(ろうれい)の母(はは)が恍惚(こうこつ)とした様子(ようす)で窓(まど)の外(そと)を見(み)ていた。

SENSE 2 - SENILITY:
An older usage referring to the confused, absent state of dementia. Made widely known by the novel 『恍惚(こうこつ)の人(ひと)』 (The Twilight Years, 1972) by 有吉佐和子(ありよしさわこ).
